淡江大學學生學習發展組個人資料蒐集、處理及利用告知事項
依個人資料保護法規範，請您於參與學生學習發展組各項活動前務必詳細閱讀本聲明書之各項內容，若您參與本組活動，表示您同意淡江大學學生學習發展組蒐集、處理、利用您與相關人員之下列個人資料，並可開始本處活動相關步驟。
一、蒐集單位：學生學習發展組
二、蒐集目的：辦理學生學習、課業輔導、獎學金申請、教學助理培訓、工讀費發放等相關活動或作業。
三、蒐集方式：透過書面、數位等合法方式取得個人資料。
四、法定之特定目的：002人事管理、036存款與匯款、078計畫、管制考核與其他研考管理、109教育或訓練行政、157調查、統計與研究分析、158學生資料管理、159學術研究、168護照、簽證及文件證明處理
五、個人資料之類別：姓名、職稱、聯絡方式等(C001)、金融帳戶(C002)、身分證號碼、護照號碼(C003)、生日、性別、國籍等(C011)、其他社會關係(C024)、居留證明文件(C033)、學校紀錄(C051)、學歷資格(C052)、學習過程、考試成績、評分評語(C057)、委員工作紀錄(C058)、工作職稱、受僱日期、工時(C061)、僱用經過(C062)、工作經驗(C064)、工資(C068)、工作表現與潛力之評估(C071)、總收入(C081)
六、個人資料處理及利用：
(一)個人資料利用之期間：
除法令或教育部另有規定外，將依學生學習發展組各項業務所需之年限進行保存。
(二)個人資料利用之地區：台灣地區(包括澎湖、金門及馬祖等地區)
(三)個人資料利用之對象：
淡江大學學生學習發展組相關人員及委任其處理本活動相關事務之必要第三人。
(四)個人資料利用之方式：
１、電子文件、紙本或其他合於當時科技之適當方式。
２、符合個資法第20條規定之利用。
七、您得依個資法規定請求查詢、閱覽、製給複製本、補充或更正、請求停止蒐集、處理或利用及請求刪除。您行使上述權利時，須填具申請表並檢具身分證明文件向淡江大學學生學習發展組提出申請（聯絡電話：02-26215656轉3526）。若委託他人辦理，須另出具委託書並同時提供受託人身分證明文件以供核對。
若申請人不符前述規定，本校得請申請人補充資料，以為憑辦。
八、前條停止蒐集、處理、利用或請求刪除個人資料之請求，不得妨礙淡江大學學生學習發展組依法所負之義務。
九、您應確認提供之個人資料，均為真實且正確；如有不實或需變更者，您應立即檢附相關證明文件送交淡江大學學生學習發展組辦理更正。
十、報名人員如不提供或未提供真實且正確完整之個人資料，導致無法進行學生事務相關業務或活動等，將無法參與本活動。
十一、淡江大學學生學習發展組得依法令或遵照主管機關、司法機關依法所為之要求，提供個人資料及相關資料。

A Notification On the Collecting, Processing, and Utilization of Personal Data
Student Learning Development Section, TKU
In compliance with the regulation on Personal Data Protection, you are required to read the following announcement carefully prior to participating in any programs. Your participation demonstrates that you agree with how Office of Student Affairs collects, processes, and utilizes the personal data of you and related people. And all related activities are allowed to begin.
A.Organization of Collection Personal Data：Student Learning Development Section, TKU
B. Purpose of Collecting Personal Data：For organizing programs, activities or related affairs, such as student learning, student guidance, scholarship application, teaching assistant training, wages giving and so on.
C. Methods of Collecting Personal Data：Legal collection methods such as written forms and digital forms to collect personal data.
D.The specific purpose: 002 Human Resource Management, 036 Deposit and Remitting, 078 Plan, control, evaluation and other research and evaluation management, 109 Education or Training Administrative, 157 Investigation, statistics and research analysis, 158 Student data administration, 159 Academic Research, 168 Passport, visa, and documents verify management
E. Code Types of identification: C001 name, title, cell phone number, C002 number and name for financial institutions’ account, C003 number of personal ID card and number of passport , C011 date of birth, gender, nationality, C024 other social relations, C033 residency proof document, C051 schools record , C052 qualification for diploma, C057 learning process, the score in the examination, comment or learning record, C058 working record of Committee members, C061 title in the work place, the date of employment, working hour, C062 employment experiences, C064 working experiences, C068 wages
F. The processing and utilizing of Personal Data：
(1)Duration：Personal data will be kept as long as required by Student Learning Development Section unless specifed by other MOE laws and regulations.
(2)Region：Taiwan, including island districts such as Peng Hu, Kingman
(3)Users：Personnel of Student Learning Development Section and related people who are in charge of programs/activities in this division.
(4)Ways of using the data：
a. Digital documents, hard copies, and any forms fit the needs
b. In compliance with Regulation #20 of Personal Data Protection Law
G. You may comply with the regulation to perform the following: searching, reading, reproducing, mending, correcting, and requesting to stop the data collecting, processing, or utilizing and being deleted. In order to exercise your rights, an application form needs to be completed with your ID and reasons and submitted to the Student Learning Development Section, Tamkang University. If the application is done by a third party, a Power-of-attorney is required.（Contact number：02-26215656 Ext. 3526）
H. The above-mentioned request should not hinder the Student Learning Development Section from practicing her obligations.
I. You should confirm on the accuracy of all your data. Provided any action is needed to bring about the accuracy, you should file a request with related information immediately.
J. False or inaccurate data will lead to inabilities of participating any programs/activities on the subject party.
K. Abided by the Personal Data Protection Law, Student Learning Development Section of Tamkang University should answer to the regulating authorities and Institutes of Jurisdiction with information in question.
